3、由监护人签字的告家长书
4、由本人签字的初试承诺书
5、歌艺展示与舞蹈展示的伴奏音乐
请转码成MP3格式后拷贝在U盘里带至初试会场

注意事项：
1、整个面试会场除我们安排的拍照、摄影和录音以外，不接受任何其他第三方的拍照、摄像和录音，请您不要自行或者安排他人开展上述行为（包括请勿使用手机进行拍照、摄像和录音）；进入面试会场请勿携带手机，或者保持手机关机状态。
2、监护人可陪同面试者前来接受面试，但监护人不得进入面试会场。
注：未满16周岁的面试者必须由监护人陪同前来参加初试
3、前来面试的选手请自备干粮和水
4、面试结果将在当天当场公布，完成面试者回到等候区等候结果公布，通过初试的选手须签订终试意向书；如果拒绝签署的不能进入终试环节。
5、面试对服装无特别要求，但不得穿着奇装异服或过度暴露的服装（例如
COSPLAY服装、比基尼泳衣等）。由于初试有舞蹈展示环节，请穿着便于舞蹈的服装和鞋子。
6、面试者应化淡妆或素颜接受面试，请不要化浓妆，以免影响面试效果。
