篇一：收养申请书的内容
收养申请书的内容必须包括收养人年龄、何时结婚、何原因无子女、婴儿性别、出生时间、收养人身体状况如何；家庭经济状况如何；有无抚养教育小孩能力；收养目的；收养后视亲生儿女对待，不虐待、不遗弃，依法承担法律责任等保证，特此证明材料全部完善后，由社会事务科审查材料合格后在申请书上盖章。
如何收养小孩
（一）、必备材料：单位证明、个人申请、户口本、结婚证、身份证。
（二）、办理程序：到居委会领取证明表一式两份（下列另有规定的除外），逐级审核后报区计生局审批。
（三）、收养条件：
1、收养人不得违反计划生育；
2、收养孤儿、残疾儿童或者社会福利机构抚养的查找不到生父母的弃婴，收养人不受无子女和只能收养一名孩子条件限制；
3、收养其它弃婴，必须无子女，而且只能收养一个孩子，并要出具公安机关出具的“捡拾弃婴、儿童报案证明”和街道计生科等有关部门签订不再生育合同一式四份；
4、收养非孤儿、弃婴、残疾儿童，还必须提供被收养人生父母户口本、身份证、结婚证、当地镇（街道）一级计生办出具的婚育、节育情况证明、小孩出生证、准生证（属计划外生育的，还需提供计划外生育费征收或罚款单据），并要与有关部门签订不
再生育合同一式四份。
5、患不孕症收养非社会福利机构抚养查找不到生父母的弃婴和儿童，需经市人民医院、市妇儿医院、市计划生育服务中心其中两家医院检查证明患不孕症、治疗无效后到区计划生育局领取申请表。
6、收养孤儿、残疾儿童或者社会福利机构抚养的查找不到生父母的弃婴和儿童后要求生育的，按一孩程序审批，除此之外仍按二孩程序审批。
7、1999年5月25日《中国公民收养子女登记办法》实施后违法收养的弃婴、儿童不予办理收养登记。其违法收养的弃婴、儿童送当地社会福利机构收养。
（四）被收养人的条件
1、丧失父母的孤儿是指父母已经死亡或者人民法院宣告其父母死亡的儿童。
2、查找不到生父母的弃婴和儿童，是指被其生父母遗弃，经查核其生父母身份不明或者没有下落的不满14周岁的未成年人。
3、生父母有特殊困难无力抚养的子女，是指生父母残 疾、患严重疾病或者其它原因造成经济生活极度困难，养不起孩子。

2.The petitioners are husband and wife and reside in the County of Orange, State of California, and desire to adopt ELIZABETH MENDEZ, the above-named minor child who was born in Los Angeles County, California, on May 5, 19----. The petitioners are adult persons and more than ten years older than said minor.
3.The parents entitled to sole custody of the child have placed the child directly with the petitioners for adoption and are prepared to consent to the child’s adoption by petitioners.
